How can i change my last name in wisconsin?

How much would it cost and where in the courthouse would i have to go? Im not getting married i just want mine and my daughters last name to be the same. Long story that i dont want the world to know but i hate my last name, and use her last name for everything cept the legal stuff and just want to make it legal.
