I was filmed fighting, under the influence of drugs - now the footage is in a feature film. What do I do now?

During high school I was a using drug addict and wound up getting in a fight that was actually filmed. The person who took the film was a peer of mine. They went on to art school and studied film. When they began making films they included that footage they had gathered in high school without making any effort to contact me concerning using my image & concerning the nature of the project. I've seen the trailer/preview for the film and there is a recognizable clip of me in the trailer. Do I have grounds for a lawsuit for defamation of character? I'm not a leader in a church, I have recovered from drug addiction and have turned my life around 180 degrees since then. I actually saw the filmmaker in my community and asked him about the film, asking to see it, since I had heard it included local footage of fights. Despite giving me his card & saying it would be fine if I called, he has not answered my phone calls or returned any of my messages. What is my next step? Do I have a case for defamation of character or does this fall into another category? Should I write a cease & desist letter/email? Thanks!!!
